A simple customizable web form built using USWDS Form.io Template Module.

To produce the form, a JSON schema is ingested where it transforms each field to a Form.io component. The form can support the following question/field types:
- Short text answer
- Multiple-choice question
- Multi-select question
- Numeric field
- List of free-text entries
- True or False
- Date/time entry
- Table / Datagrid containing sub-fields
- Email address
- Website or link
Once submitted, the form data is converted into structured JSON data ready to be shared. The output can copied, downloaded as a JSON file, emailed, and sent via issue or pull request to the GitHub repository.
This form is designed to be based on any JSON schema, making it highly customizable and adaptable to a wide range of use cases. Using schema.json, developers can generate forms with question types, validation rules, and value structures. Because the form is schema-based rather than hardcoded, it can be extended to support nearly any data collection workflow, from simple intake forms to more complex structured submissions.
